The only reason he didn't pass away at this facility is because I ended up calling 9-1-1 from inside the facility and had him taken to the hospital. In addition they left him at the dialysis center, called me at 12 in the morning and asked where my dad was!!!! Neglect! Neglect! Neglect! Do not allow your loved ones to come to this facility or any! Take care of them at your home where they are loved and cared for!

This facility is new and beautiful...please do not be fooled by their appearance and DO NOT BRING YOUR LOVED ONES HERE!! The Care is Horrible!! My Elderly Mother was there for IV antibiotics for a severe UTI and Diarrhea. While taking medications to control her diarrhea, they gave her stool softeners every day!! It was a week before we found this out. They said it was "routine". Really??? You've got to be a total idiot to give a patient with severe diarrhea stool softeners!! Selena and Janet from admissions caused me emotional distress, more depression even after knowing what I had gone through and crying to them twice . I'm glad to not have returned to a place where they treat you like absolute crap starting from the office. They gave up my bed after going to the hospital for getting sick and mentioned there was no more beds available but their sister facility would take me in for a week meanwhile something opened up, a week passed and I got excuses after excuses . There are no phones in the room, just a phone shared by the building. 2 phones for the whole place, we believe. Getting through is difficult, even to get an answer at the receptionist. During the time our elderly family member was there, the tv was out (his main diversion), the phones went out, and there was a COVID lockdown. Visiting hours are limited. I would not recommend this facility. Unfortunately my relative was in this facility for 3 weeks. The staff nurses and administrators lacked assessment skills and should have moved my relative back to acute care. We met with and pleaded with them and they just kept procrastinating. My relative of course got worse every day until he was found unresponsive. They finally moved him to the hospital, where he was intubated and died a day later. Do not be fooled by Trellis's beauty. I had taken my father for rehabilitation a few months ago and everything was great! The nurses were the knowledgeable, caring and professional. These particular nurses were 100% about the patient and helping them regain a sense of normalcy. I have to say they did an awesome job. Disgusting behavior from the staff. Talking and laughing about patients and their illnesses right outside their doors. Refuses medication even though it has been authorized. Uncaring, unprofessional, and frankly abhorrent. Everyone needs to lose their jobs
Update on the review I posted, we had to seek out assistance ourselves, as the referrals for home health care never materialized. Almost a week and a half after discharge, we were still trying to straighten out the mess. Along with never receiving a wheelchair or other supplies, that we were promised, he came home with a pressure sore and MRSA.
