Mother was put in this location after her surgery. She was given physical therapy everyday and taken care of but when she arrived home her health insurance was canceled, and someone tried to sign her up for other health care options when we told them many times she does not qualify! She also complained about a nurse who works at night who ignores patients in pain. Finally a nurse helped her at 3am! Not ok!! Do better for patients!

This facility is one giant OSHA violation. At any given time, there are numerous unresolved spills or trails of dripped liquids all over the floors. The staff either walks around or steps over the spills and trails. If the issue is pointed out to a staff member, they apologize with a brush-off attitude andvwalk away or they might put a Wet Floor sign. Behind the facility, which can be seen from the street, are dilapidated sheds and numerous weeds growing through the concrete.

After being here a month I would have to say that I have had more bad days than good. The staff is friendly and easy to get along with but on a professional level they seem to be lacking. It is tough for me because I now need help with normal activities that people would generally take for granted. Some CNAs did show empathy and genuinely seemed to care but it did not feel like it was a universal idea. The food is not great. So I have my mother in law there for the past week. The couple times I've seen her, she has been in a disarray. She's basically drugged up all day. They make her take sleeping medication when she never had to take it before. Her issue is just a fracture. I get giving her pain medication but she never asks for it and they basically give it to her like clock work. The disorganization of this institution is appalling. It took four days from the day of transfer from the hospital for me to be able to speak to my father (I am out of state). The charge nurse told me he refused a portion of the medicine he was prescribed, and another nurse told my mom he was taking all medication. After several calls I still couldn't figure out who was actually responsible for my fathers care. I'm giving 2 stars for the friendly front desk lady and one of the CNAs that was actually nice and helpful. It's a shame that all the patients that get admitted here do not receive the care they need. They are neglected, call lights are rarely ever responded to, they're giving insulin shots to patients by mistake, patients are left in their beds soaked in urine or with vomit all over them. I get that they may be short on CNAs but why won't the nurses step up? They obviously don't give a damn about these patients. It's a scam... read more

Please update the website. The rooms are not up to date, they are 3 patients per room, no problem with that, the TV, not all the TV work 100%, I had 2 controls for the TV, one to turn on and off and volume up and down, the other to change channels, air conditioners. About 25% of the rooms have ac, it took them 5 days to put me a wrist band and the rest of the patients that were here before me and not even adjusted, kept falling out, I had no problems with the CNA, LNA, PT and assisants, they did there best.

Every single staff member that I encountered was rude and unprofessional. The entire facility is unorganized and very dirty inside. The staff failed to give my family member their meds on time/ at all. Overall, this facility didn't help rehabilitate/ take care of my loved one. In fact, my loved one got worse after staying here. Let's just say, during my last visit there, TWO people within this health care center had to call 911 to get themselves out. It's that horrible in there. Don't send your loved ones here. Don't send your loved ones here unless you have a death wish for them. This place is revolting and that's putting it lightly. My Aunt was in here for one week and she's back in the hospital now. Her health declined rapidly in the week she was here and she's in worse shape now. I get everyone is doing the best they can but I feel like there are zero fucks given at this facility.
