Get directionsThis facility talks like they really do work for you. My brother is suffering there. First, they have not supplied him with a hospital bed and he fell from their inadequate bed. Thankfully he was not hurt. Second, I have been trying to reach a health professional to discuss the limited physical therapy that they provide which is the keystone of my brother's therapy need. Right now, he receives one session a day for half hour to an hour 5 or 6 days a week. The rest of the time he spends unhappily in bed with no way to h... read more

I'll be sure to get the right people in the room so that we can improve his experience and get him on the fast track to recovery. JJ, Administrator
Place is not managed well. They are all unorganized. Staff is underwhelming and not knowledgeable. Looks like they all took a 3 month class to be in there position. The Doctor here doesn't know patients at all or the medicine he is distributing. All that schooling and all you can fix is a cold. Pathetic! Get yourself more Knowledgeable staff. I asked to see my mom and they didn't even know she was in the emergency room from there negligence! Save yourself sometime and go to Kindrid.
Business response:
Dan, We're very sorry to hear about this negative experience with us. I'd like to assure you and others that our Registered Nurses, CNAs, and therapists are all licensed have have undergone the necessary training for their positions. In addition, our Director of Staff Development provides our staff with regular, in-person trainings as well as access to an online education platform for continuing education. That being said, there is always room for us to improve. I'd live to speak to you by phone or in person so that I can be... read more
Talking about being FRUSTRATED WITH THIS FACILITY. These people tell me twice about my mom being discharged and NO ONE returns my calls/emails to give me any discharge follow-up info (after leaving 3 phone messages/email. I will write another review when my mom is discharge!
Business response:

The conditions of the staffing at Sage Creek Acute is deplorable I don't know how the company can live with itself knowing that it's understaffed and not considering the patients let alone the nurses and the CNA nurses. All I can think of is that sage Creek must be in dire need of money therefore cutting staff at the sacrifice of the patients who are trying to get themselves better. It's pathetic, when the paitient calls for bathroom help and it takes close to 15 minutes for someone to acknowledge and then left there for... read more

Beautiful building, private rooms and bathrooms. Admitted November 8th under Medicare which pays 100% for 20 days. Coincidentally the exact same amount of time therapy thought my treatment would take. Wasnt allowed to use my power chair with no seatbelt, gave them info on what was needed, they never faxed it so i was stuck in my room for 3 weeks. Shout out to Mimi, Stephen and Ben. Thank you guys so much! Raul and Keylee thank you too. CNA's were ok except Tina, she's lazy and does everything half ass. My shower days were ... read more

Beautiful, clean facility, private rooms, friendly staff....but that's all I can say that's positive. If you or a loved one has to go to rehab, don't go here.

My 83 yr old mother was there 26 days. If you ever have to put anyone in this facility, please do what I did and keep a journal. My advice is to not put anyone there. Except for the physical therapy department which deserves 10+ stars, her experience was horrible! I spoke with just about every member of management to resolve issues to no avail. I was told, "I will check into that". Nothing was done, nobody got back to me. Unfortunately, I cannot say much more at this time. Shout out to Jesus PT, Vickie OT, Neil PT,... read more

My 84 year old Mother was at Sage Creek fr 11 days and the experience was unacceptable when it came to many of the nurses and the kitchen. My mother was there for swallowing issues,CHF, is a diabetic an salt restricted. The staff was clueless and heartless. I do not believe that the nursing staff ever read her chart. She did have one good nurse, Raul. Even though she and I filled out menu requests, they were wrong 95% of the time. I wish I had taken a photo of the tuna floating in milk! I am sure my Mother wasn't super nice, ... read more

I was at Sage Creek for 31 days. I tried to leave 3 times but was talked out of it each time. I have no family in the state to be my advocate. I was in severe pain and quite often felt I was in a torture chamber because my pain pills were very often more than 1 1/2 hour late. They messed up on my prescription for 48 hours one time and in the time period I should have 12 pills, I think I got 3. Some of the aides wiped me wrong(I kept complaining to the social worker) and I got a urinary infection. They took a specimen ... read more
