PLEASE do not have your loved one go to this facility!! St. Rose Sienna sent my brother here as the only place that would accept him and the Medicare timeline for inpatient was up. He had a brain tumor, and after three inpatients, two rehabs, and one discharge to home in two months WITHOUT DYING.....he died here in less than TWO DAYS!!! We were going to take him home on hospice in a few days. He had four nieces and nephew coming the next day to see him from out of state. . NEEEDS TO GO OUT OF BUSINESS!!! THIS PLACE WILL PUT YOUR LOVED ONES IN DANGER!!!! My father was sent here from Mountain View hospital for his post acute therapy!! He did not even last 14 hours in this trash sage creek facility. When we arrived they did not have his special bed he needed for his bedsores and I was argued with by the staff that it was to be approved by the dr before they could order one! My mother's nurse got COVID and this nurse helps almost every single person in this facility (they are severely understaffed). It's spread to 7 people throughout the building now. There is something wrong with how they handle COVID here. Additionally, my mom was late to her doctor's appointment because no one told her that the person who was going to drive her got sick, even though management knew about it the day before. I thought I was the only person unhappy with this facility, but after reading other reviews I see there are others. My husband has been in the facility since 02/09/21, and the care he received since he has been here is unethical to say the least. From no one answering when he needed help to his feeding tube spilling all over him and his bed because a nurse did not cap the tube correctly. UPDATE: We just got my mother in law home and noticed they forgot to remove her catheter. This place is a house of horrors!!! Unbelievable!!!! This place is horrible. They are so disorganized and have no clue.

My mother had to come here because St Rose Acute couldn't keep her at their facility past 14 days. She was improving and thriving so well from a hip replacement. My mother was here for 10 days and died July 3rd in this facility. I wouldn't place any of my relatives here. The only helpful person was Wendy. Everyone else was to busy in meetings. Walmart pharmacy and I have been trying to get this place to fix the prescriptions they wrote for 10 days. My Mom has been without her nebulizer medications all this time due to their incompetency. They also don't have phones in the rooms. You have to call a receptionist, she transfers you to a nurse's station, then they transfer it to a cordless phone and take it to the patient. Good luck if someone else is on the phone.
