Hi I wanted to give a shout out to sage creek. I was a patient here for rehab. Diagnosed with an auto immune disorder, I was not able to walk well. I am younger than the usual clientele. They were very respectful and very prompt with my medical needs and issues. I was here for a short amount of time, but it was very pleasant. Food is "ok", but let's face it...it's hospital type food.

I live out of state and had a lot of trouble getting in touch with someone to give me an update on my family member. Finally talked Lila who was extremely helpful!!! She was amazing and made me feel 100 % better.

After a week of battling pneumonia at St. Rose Hospital, my father was sent to Sage Creek. My father received good care there. The staff was attentive to his needs. He especially wants to thank: CeCe, Raul, Tina, the PT ladies and Alex, the van driver who stayed with my father for an extensive amount of time while my dad received a blood transfusion. My mom has been holding off on her hip surgery and finally had to get it done because she fell on the driveway. After calling numerous facilities, I found sage creek. I called and spoke with Wendy in admissions and she was so helpful and knowledgeable. Even though Sage Creek can't take my mom (because they aren't contracted with her insurance) Wendy walked me through the referral process and helped ease my mind about services that facilities like Sage Creek have to offer. The care provided at Sage Creek was exceptional. Staff was very courteous and professional. Physical therapy really took care of me well. They really made him work which made his recovery quicker than normal. The facility was very accommodating to his needs. I would highly recommend Sage Creek for rehabilitate. I would like to thank the staff again for all their exceptional care they gave.

Many months after our experience at Sage Creek, we learned more about how the rehab thing works with elderly post stroke patients. My father had been technically living in an elder group home (which is a wonderful place - True Loving Care - because my parents home is not equipped for his needs nor can my mother care for him in his current state. My father has been given many more opportunities for 2+ week rehab stays since July 2019. When he got stronger and was improving, he was transferred back to a rehab (a different one)... read more

This is a beautiful facility with extremely friendly staff from front desk, nurses, doctors to OT & PT and a kitchen staff willing to make changes to meals, with a smile. The place smells great, is meticulously clean and couldn't be a better place to heal!! They treat the patient and their family members like "family". I was a patient here for a little over ten days, and I have to say I was so pleasantly surprised. I came here from Saint Rose Hospital following a coma, etc. and had NO idea what to expect from a rehab place. I basically had to learn how to walk again. I was helped with everything I needed to, but also reinforced to do things on my own so that I could build my own strength up. The staff is amazing and I couldn't have asked for anything more.
