This is a beautiful facility with extremely friendly staff from front desk, nurses, doctors to OT & PT and a kitchen staff willing to make changes to meals, with a smile. The place smells great, is meticulously clean and couldn't be a better place to heal!! They treat the patient and their family members like "family". I was a patient here for a little over ten days, and I have to say I was so pleasantly surprised. I came here from Saint Rose Hospital following a coma, etc. and had NO idea what to expect from a rehab place. I basically had to learn how to walk again. I was helped with everything I needed to, but also reinforced to do things on my own so that I could build my own strength up. The staff is amazing and I couldn't have asked for anything more. My Mother was a patient at Sage Creek for about 3 weeks, I cannot say enough good about the care she received. The occupational and physical therapists are amazing, caring and a true asset for this facility. I highly recommend Sage Creek for anyone looking for a skilled nursing facility.

This is a new and well designed facility. My Father had brief stay at this facility after receiving a Pacemaker at St. Rose Hospital. Rather then draft a lengthy opus, I will attempt to simplify my review as follows: Layout/Design - A+, Well thought out Staff to Patient ratios - A- Staff Exp/Knowledge - A PT and OT staff are great Nutrition/Diet - A Cleanliness - B, Need better attention to detail regarding debris on floor and items left lying around. Sage Creek and it's staff was a god send to my Mom and I. She ended up spending 40 days there. This was a life changing and totally new experience for the family. And with the expertise and guidance of the Administrator Clint Nielson, our journey was smooth and much easier to deal with. He is an amazing person, his staff and Physical Therapist are All Top Notch. We thank- you all very much ! JoAnne and Debbie

I can not say enough positive things regarding this facility! My cousin chose this facility for a rehab from my aunt. During this scary time the director of the facility, Clint told my cousin we got you and helped her to navigate the crazy world. He is a leader of an amazing staff and we cant thank him enough for his guidence during her time at his facility

This place is better than most. The caregivers are very caring. However, information provided by family is constantly missed between physician and nurse.

My 89 year old mother was a patient at Sage Creek. The nursing staff is responsive, helpful and professional. I've had family in other facilities and the overall care here was far superior. This is a new small facility that provides short term rehab - it is across the street from St Rose Siena. I highly recommend this Rehab Facility.

I would highly recommend you do not take your loved ones there the nursing staff do not clean the patients at least they didn't cleaning my mom well at all and my mother's in declining since she's been there they say they're gonna do something and they don't and they refuse to give me any information on my mother's health on her well-being and they say it's because she said she told them not to give me any they know who I am.
