Get directionsBEWARE!!!! I understand this time we are in is a challenge for everyone, however when you send your loved one to get rehabilitated you would HOPE that there is EXTRA care ESPECIALLY WHEN FAMILY MEMBERS ARE NOT ALLOWED IN THE FACILITY. My father was sent to this facility to be cared for and looked after ....well not so!!! He was put into diapers (which wasn't needed) and left in his CRAP and URINE for hours!! Which cause an infection on his back side He wasn't given a shower OR sponge bath until a week later. When finally... read more
Think twice before bringing your loved one to West Valley Post Acute. Our loved one was 89 years old, suffered from Dementia so she was confused & didn't want to stay put, was an extreme fall risk, & they put her in a wheelchair with only one working wheel brake. In the dining rooms/activity rooms, when a resident needed something or wanted to use the bathroom, many times the residents were ignored, much like when the residents were in their bedrooms & needed something or wanted to use the bathroom. They had our loved one in... read more
My brother passed away 2 weeks ago I was notified only on a phone call about his death and was told to remove his body ASAP which was shocking to me. I was unable to visit him since march due to covid 19 and was not able to visit him . this place is only the money making for themselves by ignoring the patience when I saw my brother body it shock me to see his face which tell me that he was not taking care off and was treated like dirt. never clean him and not to notified me about his condition this place violate every single... read more
Oh my gosh!!!!! if you want to kill your family member go ahead and let them stay here at this facility. There's so much neglect it's ridiculous. It's the most mismanaged substandard facility I've ever been in in my whole life. They killed my Parent. My Parent died of a cubitus sore so large that it was not possible to even heal at all. There are so many other places .. none of them are great but this one in particular is horrible! Watch out for this one doctor his last name Dr.Shaaraway and Dr. Hirsch...Two of the worst med... read more
Two stars for trying, I ignored the warning. Mom was admitted there 3-4 days before I could get her discharged. When first transported and placed in bed, she had no blanket covering for 20-30 minutes. I asked twice! That was my first hint, Mom is 97, btw.
25 "Deficiencies" in the past 3 years. My mother-in-law was in for two days and we got her out...Stay Away!!!!
My father has been in this facility for the past three years. He's not mobile enough to take care of himself. The first two years, care and food was amazing but despite the cleaniness everything else has gone downafter it went under new management. I visit as often as I can and suggest the same to anyone that has a loved one there. Only a few care takers have a heart for these helpless elders. I've seen things with my own eyes.
I am fed up with this place , they are the most unorganized facility, I just can't take it anymore! I am tired of calling, texting & going there to ask for simple things! They can't follow simple instructions We have repeatedly asked the dr to take care of simple situation, the nurses are clueless , all just walk around looking busy and celebrating each other's birthdays One more incident and I will report them A simple incident hasn't been solved after a month of back and forth , talking to every single nurse for each sh... read more
I was truly disappointed with this facility, they let me down in so many ways in the short time my father was there recovering from a stroke. They are extremely short staffed and so my father who couldn't walk after his stroke was often left waiting and waiting for bathroom assistance so much so that in moments of desperation he attempted it on his own and ended up falling. He was then moved to a room that was closer to the nurses station so that he would get "better" attention and thus moved to a room with 3 patients cramme... read more
