Get directionsHave been looking for a facility to help with my dads rehab just recently and was referred to willow creek from a friend. When arriving I noticed how manicured the facility was from the outside very inviting. When we walked into building we were greeted from the start felt very welcoming and comforting and my dad agreed. Staff was very hospitable and resourceful and was willing to answer any questions or concerns we may have. I would highly recommend Willow Creek Healthcare Center to anyone who is looking for a home like env... read more

This is a great health care facility. Being in the nursing business for over thirty years in GA this facility has a great staff and the care is amazing. I have seen how the staff have taken care of my friends' loved one and if my parents were still living this is where they would be. I'm very picky and stern about taking care of our elderly to be respectful and loving to these great people!! Thank you Willow Creek for being one of these great facilities!!

My husband has been in the facility for almost 2 months and is totally alert. Staff does the best they can, but the last 2 weeks, facility has been extremely short staffed. Food is tasty but usually cold (during his entire stay). Last 2 weeks, meal service has been late up to an hour. Rehab at this facility is excellent. I guess new Administrator is cutting costs, but patient care is suffering. God help the people who are not alert or have no family to advocate for them.

Hi Kathy, if you would please give the receptionist a call by pressing 0, We have lines at each station so we can give her the phone if you call. Please be patient with us as we are transitioning to a new more efficient system. Most facilities do not have room phones, as it’s been a luxury at willow creek to have them. We will get this resolved. Please call to speak with our administrator , Brett Hobbs.
Patients go into their ROOMS and just literally sit in their beds.. NOT really ANY. CARE.. For me..I am 79 years old and in my lifetime I have been in several Hospitals..AND..by far..THIS IS THE VERY WORST Health Care experienc that I have EVER HAD!! For me as a patient..I was just put in a patient room and NO CARE for 6 days.AND..I can do that at home..!! I will NEVER come back to this WILLOW CREEK HOSPITAL EVER..the CARE...is non-existant CARE..I was sooo glad to leave and go home.. Really a Terrible experience!!

Hi Verna, You were with us for 3 days before you were sent out for being non-compliant and combative with our staff. Unfortunately, we can't help those that don't want help.
After submitting a complaint to CDPH, I hope they come knocking on this facilities doors. Infection control is nonexistent in here. You walk in and there are trash bags in the patients room. Filled with briefs, used wipes, and other various items of trash. I have also noticed that employees dont take contact precautions seriously and dont gown up in their PPE. They just straight up walk into the contact room and then go on to the next patient. Some nurses abuse their patients, not physically but verbally telling them to shut... read more

The care at this facility is well below acceptable standards, and administration shows little to no effort to improve conditions. Residents are treated like numbers rather than individuals deserving of dignity and respect. A family member of mine was discharged to this facility for additional care, and our family visited every single day. Throughout our visits, staff were consistently dismissive and unresponsive. Of particular concern, a night Med-Tech was observed administering medication while on FaceTime, entering resi... read more

Our loved one was railroaded into this awful place by a local hospital. They had end stage cancer. In spite of this the admission forms stated that there was nothing that indicated the patient had less than 6 months to live; they died in three weeks. Initial Friday night admit was signed by the receptionist, with loved one stating that they wanted to get stronger and go to a relative's house. The receptionist decided this plan was unrealistic. The facility would not allow any family members to be involved in the care plannin... read more
