My father went here after having sepsis and a stage 4 cancer diagnosis. He had to have a nephrostomy tube put in at the hospital and was weakened by all the infection. We sent him here for physical therapy and to ensure he had the proper care for his nephrostomy tube and to get stronger. We were so wrong about this. His tube was never changed or cleaned even after we inquired about it and his sister (an oncology nurse for 30 years) told them the same thing and nothing was done. My Brother was at Willow Creek Healthcare from March 26th till April 19Th 2025 following a near death surgery hospital stay. If I could give negative stars I would. Let me start by saying not everybody in the nursing staff and CNA's at the facility were bad but they diffidently had some that were horrible. Night Staff and Weekend Staff being the worst. While not everyone here is a bad apple there are still horrible nurses and CNAs that work here they are negligent and unprofessional. I have witnessed numerous sad situations here patients naked dragging their selves on the floor while staff laughs at them, they leave residents sitting in urine and fecal matter all day they only change residents and do hygiene checks when family state or next shift change comes , most residents have bruises due to forcefulness movement from staff, when residents make complaints to the cna a... read more

My MIL went here to recover and receive physical therapy after knee replacement surgery. Staff was friendly. Upon walking in we were shocked at the strong scent of urine. Overwhelming. Her room was awful. Walls peeling. Dirty. Tiny tv that was very old. Food was so bad we provided food for her ourselves. It looked horrible. We even brought her poor roommate food. Slow respond time from staff. Physical therapy wasn't provided as much as promised. Don't take your loved ones here. It was gross.

Difficult to get a response, nurses do not answer the phone or respond timely to request for assistance. Each time I've tried to call and get help, they pick up the phone just to put back down so I listen to the phone ring for 15-20 minutes before giving up.

My dear family member has been here for years. In the beginning they were helpful with providing rehab, but since Covid there has been no rehab available. The staff turnover has been awful. They have injured our love one on more than one occasion. They are always very apologetic, but seriously this should not happen so routinely. This place needs to be investigated. If you love your family member look somewhere else.

First thing you need to do is turn around and head towards another direction. If you love your elderly mom, dad, grandma, or grandpa, read the first sentence again. My mom was sent to Willow Creek and in less than a week she fell twice. Within a month, a total of five falls unwitnessed. Besides the falls, there were many other things that were of concern, which the facility is oblivious to any. Birthday gift, gold earrings, gone missing. Probably on one of the worker's ears.
