Approved for Hawaii, the process is easy if you read and follow all of the steps.
Our experience was flawless. The kit arrived to us as scheduled, the video observation was seamless and the delivery to the lab via UPS was smooth. Note: just remember to take Sunday into consideration. If there’s a Sunday between your test and your departure you may not make it.
The Observation team was Great! Azova website was helpful. Extremely Disappointed that they don't have a West Coast Lab! Overnight UPS didn't get there overnight from Calif to Georgia!!! Almost didn't take our vacation!! Had to CALL late the night before flying to see if our test was finished. They didn't send the results by email until we called. Go to an airport where they have local labs available. ALSO, My friends got their test free at a local Utah Walgreens and received it back within 24 hours.
This system worked, though there were lots of steps to follow. We had a great trip to Hawaii. The benefit to us, residing a distance away from approved providers, was that we could take the tests without losing a day of work to just take the test w/in the 72 hour window required.

The lab was slow with completing our test results. I called their customer service and they finally put a rush in getting our results. We were starting to panic- got our results less than 12 hours before our flight left. The rest of the process was very easy and very organized.

I purchased this about 14 days before I traveled. Once I received the coupon, I immediately requested the PCR Kit and scheduled the video conference to do the test. There were a lot of times available. I scheduled my test for 10 am on Thursday (was leaving at 9 am Sunday). I ended up having to reschedule last minute (b/c I drank before the call) and was easily able to move the video conference to 11:30. An hour or so later, I dropped it off at UPS. The product itself, and the instructions from AZOVA, were very straightforward. Purchasing it through Costco was not...The confirmation/receipt I received from the online order (not available in my local Costco) lead me to believe I was registered for the test. You have to register through AZOVA before the test will actually be sent to you. Following receipt of the test kit itself, the process is very quick and streamlined.
