A new update to my review of this facility well to start off my friend who was here thanks for their neglect my friend has passed away And her family can not get any information about her sister it has been very heartbreaking for my friend and her family and everyone that has known her and no reason why this tragedy has accrued so I am praying that no other person has to suffer the same fate as my friend So please if you have any options please find a different facility and keep a close eye on your family that is there or ... read more

Good luck if you have a loved one here. Dirty place and the housekeeping staff does not like to clean or change linens. My so has not been bathed or groomed in 6 months

I've had family members admitted here I've had friends and family work here. There is good and bad you come across some good nurses here very few though. Henry is the best man in the building so helpful for family members and staff. DSD office is horrendous especially Araceli she is a bully to her staff she likes to pick and choose who she can step all over and harass. I renewing my review of this place my friends sister is still a resident of this so called rehab facility and they have not improved their service since my last review as a matter of fact they are worse my friend sister has been taken to the hospital again she has always having to be taken to the hospital around once a month for unknown reasons that they refuse to tell my friend This place needs to be investigated by the medical board of trustees and answer for all the issues they are putting my friend through

This facility is among the worst in this area, my father was in this facility for over a month, and they did nothing for him. His sheets from his bed were constantly dirty and ripped. When we asked any questions to the staff, we would only get rude responses. Raven a CNA is the amongst the worst in the facility. They also sell candies, chips, sodas to diabetic patients then they want to administer insulin. To staff past and present (if you are thinking about working here, don't. You will witness horrible treatment of fragile patients. You may even be asked to cover up your co workers abusing patients. A patient said Michael assured them we are fully staffed. Big lie. Lots of shifts we have no CNAs or nurses or just one for patients. Especially p.m. shift. They said my mom had an infection and supposedly they were giving her antibiotics. But when we went to visit her, there were no signs they did. And the next day when me, my dad and her sister went to visit her she was unresponsive and looked pale. We told the CNA to Check her and they did and her blood pressure was low (78/49) and they "needed a reason to take her to the hospital or they would sent her back" but when she did go to the hospital the nurses and doctors said if they knew she had an infection they would have toke... read more

DO NOT LET YOUR LOVED ONE ANYWHERE NEAR THIS PLACE! Staff is generally kind but insufficient and poorly trained. Rooms are small, filthy, and nothing works. Went in for physical therapy after hospital stay but got no effective treatment. Management is terrible. Was forced to stay 3 extra days after finding alternative private pay facility because key management took time off and there was no backup who knew how to fill out discharge paperwork. Medicare was charged unnecessarily.

Terrible Terrible Dump!!! My sister was there for two weeks and ended up with UTI, fever! Staff was all around nursing station when I went in to see my sister. She was on oxygen and was sweating and I lost my cool! Called 911 and had them come and take her out of this Hell hole!! This place should be shut down!!!
