I give this Facility a ZERO! I am a POA (Power of Attorney) for my friend and IF she went straight to her Assisted Living home from her Rehab stay instead of going here....I truly believe she would be with us today. When they first transferred her to Canyon Vista...she wanted me to take her out immediately due to the fact that the bathroom was filthy and the nurses wouldn't come when she had her call light on. The Staff (nurses, etc) never kept me informed on her progress and I tried my very best to get her out via talking... read more

I wish I could talk more about this place but unfortunately things didn't go as smoothly as I He planned for my fiancée, it didn't turn out to be a great end. My fiancé said at the time of service .no one came around for hours & was left alone. Then I arrived and no one greeted me or talk to me. Finally 10 hours later A night shift crew came in . They knew nothing it was so quiet and possibly I could say spooky . When I finally left it was 11 PM at night and my fiancé was in the room all alone . At this point he was exhaust... read more

This place is Horrible. Cafeteria staff drops off measly food couldn't fill a baby, let alone a grown up. Staff nursing are rarely found if ever. Dr sane thing good luck seeing one. Now I've been in here 1 month at least seen a therapist maybe 4x at the Most! How am I to get better with no therapy. My meds for pain are also not being given to me at right times "IF" I even get them. This is not a rehab but a NIGHTMARE . No human should be treated such a way as this. They should be shut down....

If I could give zero stars I would..my mother was sent there on January 14th 2022 after having a colon infection to build her strength up..she sat and got weaker & weaker finally I said to the nurse ..she needs PT that's why she's here she needs to get out of this bed...the longer she sat the more weaker she got. She had PT on the 26th apparently and she collapsed & died. They supposedly worked on her for 30mins...why not rush her to the hospital across the street immediately?? My dad was Refered here from Sunrise hospital after having a stroke, this place is really bad. My dad would click on the nurse remote for help they wouldn't come and help him I had to go and hunt for a CNA. They don't help you if you want to go to the restroom, they rather put a diaper, I asked if they can shower my dad the CNA said no we have a scheduled I asked can you please check and let me know she never came back. i would not put my worst enemy's dog in this place. they should be under investigation for the careless way they take care of their patients and treatment of them! this place put myself and my poor mother through hell a few years ago! i am very surprised they are even still open. when i took my mother out of this [kill center] her urine in her catheter bag was black! i barely ate, or slept for a month caring for her! i had her drinking water every 15 min. and in three days i had her urine clear. they had her on twenty differ... read more

My father was a resident there. The staff are horrible. They left my father lie in his urine and stool for hours. The facility has a strong urine odor. Staff do not practice good hand hygiene. My father developed serious life threatening urine and blood infections.

This place treated my brother terrible after his stroke ,other patients reported it to me when I wasn't there. they lost his things when they moved him from one room to the other made no effort to find them and wouldn't let me look for them. Never anyone in charge to ask for help didn't know what the condition was never knew what was going on no one ever knew !! couldn't get any help or information from anyone ,,go anywhere but here. He said he was frightened while he was in there.

Horrible experience! Not for physical therapy rehab! They just want to keep you there and drugged
