If I could leave 0 stars I would. I work for a major hospital in Minneapolis and this is by far the worst insurance we have ever dealt with. The deny everything as "pre-existing" and request medical records for dates or service outside of our patient's coverage with them, they take months to pay very little or deny claims, they forget to send claims for reprocessing when they deny incorrectly in the first place and they refuse to help with more than 3 claims at a time. BEWARE! THIS COMPANY IS NOT CAPABLE OF PROCESSING CLAIMS AND TELLING THE TRUTH. THEY CONTRADICT THEMSELVES, HANG UP ON YOU, NO ONE WITH ANY RESPONSIBILITY OR AUTHORITY AVAIBLE...NEVER AVAILABLE. PHYSICIAN'S OFFICE CALLS FOR INFORMATION WITH THE SAME RESULTS TOO. THEY ARE ABSOLUTELY FRUSTRATING TO TALK TO WITH NO RESULTS. NUMEROUS CLIENTS HAVE COMPLAINED. THIS IS HOW THEY "CONDUCT BUSINESS". HOW THEY ARE ALLOWED AS A BUSINESS TO PERFORM, OR LACK OF PERFORMING AND YET STAY IN BUSINESS...WHY?
All I can say is this insurance is pure fraud!!!! All they have done is steal my money every month. They have not paid one claim. You can never get a supervisor on the phone, the only people you can talk to is a call center in the Philippines. HORRIBLE insurance!!!!!!!

I wish I would have done my own research instead of blindly trusting a broker. This has been an absolute nightmare to deal with. My situation is the same as the other reviews here in that every single claim is met with “denied pending review of medical records to determine pre-existing condition”. My doctors never received any request for my medical records. Worst medical coverage I’ve ever had in my life!!!! They don’t pay claims even though they are covered services. Takes multiple calls for months on end with no improvement in outcome. Horrible to deal with! Lost my daughters doctor because of this horrible company not paying in a timely manner. Stay away!!!!
BEWARE!! This company exists to refuse to pay any claims. This company is a scam and should be investigated, exposed and closed. Likely some illegal activity is happening here.

Would leave 0 stars if it were an option!!!! We were told there was Emergency room coverage after deductible like a normal Ins company 80/20, now stuck with a $7000 bill because they only pay $250. I have been going back and forth with them on a separate claim from 2-13-2018!!! 16 months. That is completely absurd. They have requested medical records and then lost them, never received, etc. same as others have posted. I never get a phone call back like promised. Different answer every time I talk to someone. My partner made the mistake of selecting this plan to protect our family... a family of a beautiful little girl with a life threatening condition that needs careful monitoring. We have been met will road blocks at every turn...every invoice... every doctors appointment and blood work is not covered. Everything for her is pre existing... a child...denied coverage she needs. I am a provider. I've been trying to get some claims from January paid since March. Each time I call they said the claim is set to pay, but there is no check information. The claim has been sent back for escalation more than twice and there is still no resolve. I asked to speak with a supervisor but one is never available. I asked to receive a call from a supervisor and I have not received one. It should not take this long to receive a payment.
