In fairness, the facility looked nice and clean. It also had a lot of activities. However, the patient rooms looked to have 2 or 3 patients per room. My father died at this skilled nursing facility during his recovery. He was stuck in a Covid patient room that was very small and shared with another patient. My father had cleared Covid about a week earlier and was waiting to be placed in a new room. But none of the regular patient rooms were available. We kept waiting for an opening. If I could give zero stars I would. The care my husband received was very good UNTIL: They didn't get authorization for another 2 weeks of care and Physical Therapy, didn't tell us, and kept him for an additional 2 weeks. Now they are trying to bill us saying that the insurance rejected the claim. That's not true. The insurance wants more information. They are refusing to provide it. Outpatient Post-Op Rehab Referral So far I've been less than impressed with the way my referral to their on site services for post-op rehab as an outpatient has been handled. I had surgery on 6/2/2025 and spent a week in the hospital. After discharge, I was given a referral to a different physical therapy provider that unfortunately didn't take my insurance. Recovering from quadruple bypass surgery, I was sent here for rehabilitation, this has got to be one of the most horrific places to be housed. I came here at 5:00pm 05/16/2025 did not receive a bed until 2 hours later, once I received the bed, I discover the sheets have not been changed, the bed's alarm would not shutdown. It took another 2.5 hours before that situation was rectified. The bed is finally ready for me and the roommates are using their phones at full volume 1:00 am. Unfortunately, they deserve 0 stars for poor phone service. Completely crap phone service!! It's a hit or miss if they answer the phones here. You are on hold and and they either forget your on the line holding or hang up on you. I've called several times and no answer this is repetitive process they put you On Hold just to hang up or never pick up. Crappy customer service and no monitor of their lousy phone system. Ridiculous untrained lazy no phone service to residents.

Horrible! Just horrible. They promote physical and occupational therapy. They even have Kaiser believing it, but it's horrible...loud, constant interruptions to your rest, no sleep. Horrible.

Been trying to get ahold of my friend for over 24 hours and no one will answer the phone or when they do they just transfer me and it goes to voicemail. EXTREMELY UNSATISFIED WITH THE MULTIPLE PEOPLE AND STAFF THAT HAVE ALL ANSWERED MY CALLS HERE. No one has been able to help me at all! I do not recommend this place at all! Still haven't reached my friend who had a stroke! This is awful service!!

My father caught Covid while he was staying at this facility and died at Good Samaritan Hospital in January. They sent him to Good Samaritan Hospital with fluid in his lungs. He died 5 days later after he was admitted to the hospital. I am finally going to give my review fo this facility This was my dads third facility he stayed at.
